New clients may complete paperwork prior to their first session. The necessary paperwork is available for your convenience by clicking the buttons below ~ paperwork will open in a PDF file format. After filling out the paperwork you may submit via e-mail to erica@weinsteinwellness.com or bring to our first session. Completing the paperwork ahead of time saves time at our first session.
